MyProtection Cash Plan

MyProtection Cash Plan is an Individual Health Insurance product that provides benefits in the form of daily compensation if the Insured is hospitalized in accordance with the terms and conditions, and as long as the Policy is still valid.

Daily Inpatient Compensation Benefits

As long as the Policy is still valid and in accordance with the terms and conditions, PT Asuransi Allianz Life will provide daily hospital care benefits as follow:

    5. Dyspepsia

Claim Procedure

Claim Procedure

  • Please send required claim documents to PT Asuransi Allianz Life no later than 30 calendar days since discharged from the hospital.
  • PT Asuransi Allianz Life will pay the claim within 7 working days when the claim has met all requirements, complete, received by PT Asuransi Allianz Life Headquarter, and approved by PT Asuransi Allianz Life.

Required Claim Documents

  • Claim form that has been completely and correctly filled and signed by the Insured with medical summary section completely and correctly filled and signed by Doctor in-charge and sealed with hospital stamp.
  • Proof of payment for treatment in form of original invoice or a copy.
  • Proof of length of hospitalization.

  • Login using your user ID and password.
  • On Homepage, click "Klaim/Claim" on Menu Bar or click “Klaim Saya/My Claim” box .
  • Fill in the requested data: policy number, policyholder name, insured name, hospital name, and currency.
  • Upload required documents.
  • Mark or check the “Syarat & Ketentuan/Terms and Conditions” box
  • Click Submit button and claim registration number will appear on screen.
