Maybank Credit Shield

Maybank Credit Shield is an insurance that provides protection to Maybank Credit Card Holders when they are unable to pay the balance of the Maybank Credit Card bill due to illness or accident causing death, total permanent disability, temporary disability. Credit

Card Debt Balance on Maybank Credit Shield is all use of Maybank Credit Card facilities by Participants prior to the insured event, including auto-debit routine bills and installment programs (if any) which are billed on the last Maybank Credit Card billing statement after the insured event occurs.

Terms & Conditions Maybank Credit Shield

Entry Age:

21-59 years old
Insurance coverage period is valid until the age of 65 (on the last birthday).

Participation requirements:

Maybank Credit Cardholder.

Coverage Period:

As long as the insurance policy is active, the policy holder will still pay the policy.

Premium Payment:

Premium is a payment that must be paid by the Maybank Credit Cardholder (the “Insured”) on a monthly basis to PT Bank Maybank Indonesia Tbk. (“Policy holder”) of 0.69% calculated from the total invoice of the Insured Maybank Credit Card*. The total bill of Maybank Credit Card includes all bills in the printed month, fees, fines, and the remaining installments that have been billed (if any).

Sum insured:

Sum Insured is the amount agreed and paid by PT Alliannz Life (the “Insurer”) to the Policy holder and/or the Insured's heirs based on the Last billing balance if the Insured dies, suffers permanent total disability or temporary total Disability ("Insured Event" ) and fulfill the requirements stated in the policy certificate and/or the master policy. The last billing balance includes all costs, fines, and installments/installments that have been billed (if any) before the insured event occurs, with a maximum sum insured of IDR400 million.

Expiration of Protection:

1. The closing of the Maybank Credit Card.
2. Non-payment of premiums for 3 consecutive months.
3. Experience permanent disability or death.
4. There is a request for cancellation.

Notes:

1. The guarantor is PT Allianz Life.
2. Policyholder is PT Bank Maybank Indonesia Tbk.
3. The Insured is a Maybank Credit Cardholder who participates in the Maybank Credit Shield insurance program.
4. The insured event is death, permanent total disability, or temporary total disability.
5. The last billing balance includes all costs, fines, and installments/installments that have been billed (if any) before the insured event occurred.
6. Exceptions to the payment of benefits or sum assured are regulated in the policy certificate.

Maybank Credit Shield Claims Procedure

Claim Deadline

Written notification to Allianz no later than 30 (thirty) days after the occurrence of the insured event that gives rise to an insurance benefit.

Payment of claims will be made 5 working days from complete documents and approved payment.

Claim Terms

The Participant must complete the claim form provided by the Insurer (Allianz) as evidence that the Insurer (Allianz) believes to substantiate the claim submitted, provided that the Participant's coverage under this insurance is sufficient to prove the following documents:

1. Temporary Disability claim document
   1. Insurance claim form (Allianz).
   2. Copy of Insurance Certificate; and
   3. Maybank Credit Card billing statement containing the billing and payment of Participant premium for the last 3 months.
   4. Photocopy of the Insured's KTP.
   5. Disease diagnosis / Photocopy of medical resume.
   6. Doctor's certificate for a temporarily disabled body part.
   7. Certificate from the police if temporarily disabled due to an accident.
2. Permanent and total Disability claim documents:
   1. Insurance claim form (Allianz).
   2. Copy of Insurance Certificate; and
   3. Maybank Credit Card billing statement containing the billing and payment of Participant premium for the last 3 months.
   4. Photocopy of the Insured's KTP.
   5. Disease diagnosis / Photocopy of medical resume.
   6. A doctor's certificate for a total permanent disability.
   7. Certificate from the police if the total permanent disability is due to an accident.
