Business Entity has been established minimal 5 years
PKS with Maybank
Fill out the merchant application form
Copy of establishment and its amendment certificate
Copy of Company Registrasion Certificate (TDP), Tax Identification Number (NPWP), Trade Business License (SIUP)
Copy of ID Card, Passport, company officials' KITAS
Bank statement for the last 2 years (audited) to show company's profit
Recommendation letter from a Reseller
Company Logo

A period of cooperation with Distributor minimal 2 years
Average purchase to Distributor IDR 25 Million/month (for the last 3 months)
